Distribution Responsibilities:
 
MT-1: This memo should be completed by the Extern and delivered to each Master Teacher.
MT-2: This memo should be completed by the Extern and delivered to the Master Teacher in the first week of instruction. A copy should be completed by the Master Teacher and returned via the Extern to the PDD Coordinator.
MT-3: Extern completes or makes 3 copies of schedule of each Master Teacher’s schedule in its entirety. One copy is returned to PDD Coordinator; one copy is returned to University Supervisor/Program Advisor and the candidate keeps one copy. Print all information clearly! Indicate which classes you are observing in by checking the appropriate box.
MT-4: Two copies for each Master Teacher should be completed by the Extern and delivered to each Master Teacher in the sixth week of instruction. A signed copy should be returned to the PDD Coordinator.
MT-5: Extern submits one copy signed by each Master Teacher and returns forms to the PDD Coordinator.
