Distribution Responsibilities:
PRE-Alert: Copies of this form should be provided to the candidate and SECTEP Coordinator. The SECTEP Coordinator will forward a copy to the Subject Area Advisor of the student's teaching content area and Chair of Secondary Education.
PRE-1: This form is to be completed by candidates pursuing a single subject credential at Cal State Fullerton. The candidate is responsible for tracking his/her advisement through the program, and this form must accompany the student's application to the program. It is also a class requirement of all EDSC 310 students.
PRE-2: Candidate submits to EDSC 310 Instructor. Candidate should keep a copy for his/her records.
PRE-3: Candidate submits to EDSC 310 Instructor. Candidate should keep a copy for his/her record.
PRE-4: Candidate submits to EDSC 310 Instructor. Candidate should keep a copy for his/her record.
PRE-5: This form is to be used during the candidate interview for admission to the program by the subject area advisor.
PRE-6: This form is to be completed by each candidate pursuing a single subject credential at Cal State Fullerton. It will be submitted to the PDD Coordinator during Pre-Orientation.
   
Course Equivalency Petition: See directions on the form.
 
Note: Candidates should always place a completed/signed copy of all forms in their Teaching Assessment Documentation (TAD).
