Level II Credential Application Instructions

1. Complete and print the application;
2. Download and print the Verification of Experience, CL-41EXP form for employer to complete. (Two years of successful experience while holding appropriate credential)
3. Mail or Deliver Application and Verification of Experience form with $25.00 processing fee (check made payable to CSUF) to the following address:

   CSUF Credential Preparation Center
   California State University, Fullerton
   2600 East Nutwood Avenue, Suite 740,
   Fullerton, CA 92831

4. CTC will e-mail you their instructions once CSUF has submitted your recommendation via online.

If your Internet provider is AOL, please refer to the instruction below once CSUF has notified you of their electronic recommendation submittal to CTC:

1. Go Online and type in (or copy and paste) the following web address: https://teachercred.ctc.ca.gov/cctc_phase3/InteractionMgr?interactionmgr.interaction=Enter_ApplicantLogin
2. The page titled Credential Recommendation should come up
3. In the section titled Applicant Log In, enter social security number and date of birth
4. Continue to follow all instructions, entering all necessary information
5. Print out Confirmation Page, as this is your receipt
